This document provides guidance for a placement practice that involves the creation of a "triage" team. A triage team brings together the key partners on a case, including family, to make a thoughtful family-based placement that is well-supported. 

This work plan template can be used to identify, schedule and assign specific tasks that need to be completed to implement new Family First strategies.

This roadmap provides counties that are participating in the Family First Readiness effort with an overview of the initiative and the supports provided by their readiness consultant during each phase of work.
